Off-the-Cuff Puzzle for Barbarany

This "early-week level" puzzle was inspired by recent events in our family's personal life, but was constructed in a way that no inside information is required to solve it. To illustrate the puzzle [and provide a hint to 27-Across], we show you Dr. Alicia K.
